I understand that making false claims for postal losses is a crime in accordance with 18 U.S.C.288. Persons making false claims for postal losses are also subject to civil penalties in accordance to 31 U.S.C. 3801-3812.

I certify the following in reference to the merchandise in question:

1. I have not received the below-described parcel.
2. To the best of my knowledge, the below-described parcel was not delivered to my address.
3. I have no knowledge of the whereabouts of the parcel.
4. I will cooperate with the US Postal Service and/or FedEx in any investigation of theft or fraud.
5. This is a claim for replacement of the lost parcel.
6. Should the original parcel be received after reshipment is made, it will be immediately returned.
